    What is Benign prostatic hyperplasia (BPH)?

    Benign prostatic hyperplasia (BPH), is enlargement of the prostate.

    It occurs when the cells of the prostate gland begin to multiply. These additional cells cause your prostate gland to swell, which squeezes the urethra and limits the flow of urine. There are two main growth periods as a man ages: The first occurs early in puberty, when the prostate doubles in size and the second phase of growth begins around 25 years of age and continues during most of a man’s life. BPH is common in men over 50.

      Causes of Benign prostatic hyperplasia (BPH)

      The benign prostatic hyperplasia occurs mainly in older men and does not develop in men whose testicles were removed before puberty. Any family history of prostate problems or abnormalities in the testicles may raise the risk for BPH.

      Symptoms of BPH

      Common symptoms include:

      • Incomplete bladder emptying
      • Need to urinate two or more times during the night
      • Incontinence, or leakage of urine
      • Weak/ slowed/ delayed urinary stream
      • Sudden urge to urinate
      • Painful urination
      • Blood in the urine

      Men 40 years and older

      Men with a family history of benign prostatic hyperplasia.

      How to diagnose Benign Prostatic Hyperplasia (BPH)?

      Diagnostic tests may vary from patient to patient.

      Urine flow study:- The patient voluntarily empties his bladder and the amount of flow is measured. 

      Digital rectal examination (DRE):- The physician inserts a gloved finger into the rectum (located next to the prostate) and feels the back of the prostate.

      Prostate-specific antigen (PSA) blood test:- Elevated levels of PSA in the blood may sometimes be an indicator of prostate cancer.

      Post-void residual:- This tests the amount of urine left in your bladder after urination.

      Cystoscopy:- In this a doctor examines the urethra and bladder by inserting a tiny lighted scope into the urethra.

      Trans-abdominal ultrasound:- Done to measure the size of the prostate and the amount of urine left in the bladder after urination.

      Prostate magnetic resonance imaging (MRI):- MRI provides views of the entire prostate with excellent soft tissue contrast.

      Benign prostatic hyperplasia (BPH) - Treatments

      Treatment of BPH can begin with self-care and lifestyle adjustments.

      Active Surveillance :- Often, BPH will only require active surveillance with regular visits to your urologist. Diet and exercise are often recommended as a way to prevent symptoms.

      Surgery: There are different types of surgery that can be used to remove the prostate tissue that blocks the flow of urine, including:

      Transurethral resection of the prostate (TURP): The urologist removes the tissue blocking the urethra with a special instrument. 

      Transurethral incision of the prostate (TUIP): The urologist makes two small cuts in the bladder neck (where the urethra and bladder join) and in the prostate to widen the urethra to improve urine flow.

      Transurethral electro-vaporization: It uses electrical energy applied through an electrode to rapidly heat prostate tissue, turning the tissue cells into steam. This allows the doctor to vaporize an area of the enlarged tissue and relieve urinary blockage.

      The green light laser: This procedure removes prostate tissue with a laser.
